I accept private (non-insurance) and insurance payments. Please contact me for private payment rates. I accept cash, personal checks, and credit cards, including Health Savings cards.

A formal diagnosis, which I can provide, will be submitted to your insurance company in order for payment of therapy services. Insurance will not cover Consulting Services.

I am "in network" with Traditional Medicare and United only. I do not accept Medicare Advantage Plans or Medicaid, including Medicaid that is secondary to Medicare.

If you will be using an "out of network" insurance plan, you will be expected to pay the private pay rate at the time of service. I can provide a receipt for you to submit to your insurance for reimbursement. Please note, reimbursement is typically less when using an "out of network" provider than when using an "in-network" provider.
